PCN Team

Clinicians and staff from the Primary Care Network (PCN) team assist and hold clinics at The Quarter Jack Surgery. These include GP, MSK, Health Coach and Social Prescriber sessions. The PCN Team that currently work at our surgery are:

 

Adrian  – MSK First Contact Practitioner – Musculoskeletal Physiotherapists are Health Professionals with specialist training in the diagnosis and treatment of musculoskeletal conditions helping you to become more mobile, active and productive in your daily life by helping you restore movement and function..

Adrian sees and assesses patients with soft tissue, muscle and join pains.

 

Chris  – Social Prescriber – Social prescribing is a service provided by your local GP surgery and is a way in which you can be helped to link with a range of local, non-medical activities, opportunities and community support that can improve your health and help you to ‘live life to the full’.

Chris helps patients to improve their health, wellbeing and social welfare by signposting patients to community services run by Charites and the council. Chris sees patient over the age of 13.

 

Lesley – Health Coach – A health coach’s main role is to help people to take an active role in looking after their own health and wellbeing. Particularly people with long-term conditions that can be managed through lifestyle changes. This can be achieved through setting goals and targets, establishing healthier habits and increasing peoples knowledge, skills and confidence.

Lesley works with patients to make positive lifestyle changes to better their overall health.

 

Dee  – Pharmacist – Clinical pharmacists are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach.

Dee conducts medication reviews with patients as well as helping our pharmacy team process prescriptions.

 

David – Paramedic – A paramedic practitioner is trained to independently provide care that does not require the intervention of a doctor at that time. The paramedic has access to the full GP record. They report directly back to the GP with the outcome of the visit and any updates on any treatment and medication that was given. The aim of the service is to minimise the disruption of home visits to GP surgery schedules and reduce the number of A&E attendances.
