If you have a cardiovascular condition you will be invited for a review at least once a year. Depending on your condition you might be required to attend for more frequent reviews. You will be asked to submit 7 days home blood pressure readings (using the online eConsult platform to submit these), your current weight and asked to attend for a blood test. The results from these checks are reviewed and you may be invited for review with the cardiac nurse.
You will normally be invited for your reviews by text if we hold a mobile number for you, or by letter or phone if not.
