Patients aged 40 – 74 who do not have certain pre-existing conditions are invited for NHS Healthchecks . The check must take place before your 75th birthday. You are entitled to a NHS Healthcheck every 5 years if you meet the following criteria:
I am an NHS Dorset resident aged between 40 and 74.
I have NOT already been diagnosed or treated for any of the following:
Diabetes
CHD
Heart Failure
Atrial Fibrillation
Hypertension
Stroke/TIA
Renal disease/CKD
Familial hypercholesterolemia
Peripheral Arterial Disease
I am not receiving palliative care treatment or taking statins.
I have not had a NHS Healthcheck in the last 5 years.
