SystmOnline
This is a simple and easy way to book, view or cancel appointments and request prescriptions at any time of the day or night.
You must first register your details with the practice so that you can be provided with a unique username and password. You will only be able to register yourself – we will not allow anyone else to register your details and you will not be able to register details of anyone else, including relatives.
The service is available if you are 16 years of age or over. Your personal information is kept safe and secure when using SystmOnline.
To register for SystmOnline
- Fill in the online SystmOnline registration form.
- Please bring 2 forms of ID (including 1 x photo ID ie passport/driving licence) to reception and ask to register for SystmOnline.
- You will be given a unique username and password – it is highly recommended that you change your password to something more memorable when you first log on.
- Go to the SystmOnline website.
- Login using your username and password.
Using SystmOnline
There are a selection of appointments available through SystmOnline. Please take care if you are booking nurse appointments. Only the procedure indicated will be able to take place in these appointments – nurses are trained in differing specialties and booking with the ‘wrong’ nurse will waste your time and theirs. If your required appointment is not available please telephone your Registered GP’s PA who is trained to offer the most appropriate appointment for your needs.
PLEASE NOTE: This link takes you to the SystmOnline website which we are not permitted to edit. To register for online services please read the information leaflet and attend the surgery with your completed application form and 2 forms of ID. Access to patients under 16 or to another patient’s record is not permitted. Access to the Detailed Coded Record and the Full Record is not yet available at The Quarter Jack Surgery.
