Registration with the practice
If you wish to register you will need to complete a Registration Form GMS1. You can register with us if you live within the practice area - if in doubt please telephone the surgery.
When you register you will also need to complete a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. You may print the questionnaire from the link below.
Please complete one form for each person applying to register and return at least 2 working days before your first appointment. If you are registering a child under 5 there will be an additional form to complete at the surgery.
As a newly registered patient you are invited to attend the surgery for a 'New Patient Check'. This appointment would normally be with one of our practice nurses. If however you require medication or would like to see a doctor for any other reason, then you should see your doctor.
Dr Neil Rennie joins the surgery on February 1st 2010 and will be taking new registrations from that date. Please see below - PERSONAL MESSAGE FROM DR NEIL RENNIE
Please note, we will need to see a form of photo ID (ie passport/driving licence) and current proof of address (ie utility bill/letting agency letter) when we accept your application to register.
Please collect a New Patient Pack from reception which will contain important information regarding the Summary Care Record.

PERSONAL MESSAGE FROM DR NEIL RENNIE
Hello to everyone linked to The Quarter Jack Surgery. I joined the practice on the 1st February 2010 and I am really looking forward to getting to know you all. As your GP I thought you might like to know a little about me, especially since for some of you, the doctor-patient relationship we develop, will last for more than 30 years - quite a thought really!
I was brought up in Aberdeen and went to Aberdeen University where I graduated as a doctor in 2000. Since then I have worked in both Scotland and England as well as New Zealand. I initially trained in General Medicine, specialising in Gastroenterology having become a member of the Royal College of Physicians in 2005. Part of this training was undertaken in Poole and Portsmouth Hospitals so I already know this part of the coast quite well.
In 2007 I switched to general practice and I completed my training in the summer of 2009. I moved to general practice because I was attracted by the variety of clinical encounters that it offers. I also enjoy getting to know people and understanding their concerns as this, in my opinion, is the best way to be a good doctor and provide good medical care.
I hope you will find me as someone who is easy to speak to and who treats you as a person. I'm certainly not a machine and though I work hard, I have plenty of interests outside of work, especially football and golf. Don't worry though, I only play golf at the weekend!
