Registration with the practice
If you wish to register you will need to complete a Registration Form GMS1. You can register with us if you live within the practice area - if in doubt please telephone the surgery.
When you register you will also need to complete a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. You may print the questionnaire from the link below.
Please complete one form for each person applying to register and return at least 2 working days before your first appointment. If you are registering a child under 5 there will be an additional form to complete at the surgery.
As a newly registered patient you are invited to attend the surgery for a 'New Patient Check'. This appointment would normally be with one of our practice nurses. If however you require medication or would like to see a doctor for any other reason, then you should see your doctor.
Please note, we will need to see a form of photo ID (ie passport/driving licence) and current proof of address (ie utility bill/letting agency letter) when we accept your application to register.
Please collect a New Patient Pack from reception which will contain important information regarding the Summary Care Record.
